We are always accepting referrals from physicians for diagnosis and follow up of HPS patients. Appointments are usually available within 3-4 weeks once the referral is received.
If you would like to refer a patient to our clinic please fill out the referral form for either the Toronto or the Montréal HPS clinic and fax it to the number below.
